TIPA Questionnaire – Incident Management
This questionnaire should be used to assess the maturity of the incident management process. This file contains a short description of the Incident Management process (in term of purpose, outcomes and base practices) with a full set of questions related to the day-to-day activities (like the incident recording or the incident escalation) and to the quality aspects of the process. Print one exemplar of this questionnaire before each interview, and use it during the interview to write down your evidences and findings and to rate the process practices.
TIPA Rating Sheet – Incident Management
Use this file after the interviews to combine your interview ratings and determine the process ratings, to collect the evidences, to perform your SWOT analysis, to determine improvement recommendations and finally, to generate your Incident Management process profile. This example file is a limited version, only usable for combining the ratings of two interviews.
